How can Loreto help us with our Sr. family member?
Loreto is here to help clients and their families navigate the complex healthcare system with the medical and cognitive challenges they face as they age. Loreto’s primary focus is to promote health and safety, by properly managing all aspects of our client’s healthcare holistically. Loreto is not a home health nursing agency and does not provide direct, hands-on nursing care. Rather, we coordinate and manage our client’s individual care needs.
What role do you play in the care of our Sr. family member day-to-day?
If you are caring for a spouse or an aging parent, you know it is complicated and filled with daily challenges. Loreto Company provides guidance and support for care providers struggling to get through the day by guiding them through complicated decisions and providing resources and recommendations. We review our client’s needs, provide resources that support them in their home or living community, and empower families with the tools they need. We aim to alleviate caregiver stress and burden from family members, so the family member can return to being just that.
How does payment work?
Loreto invoices for our services bi-weekly. Itemized billing will appear on all invoices.
Billing and charges will reflect time spent, including face-to-face time with clients/family, phone conversations, emails, texts, care coordination with members of the interdisciplinary care team, accompaniment to medical appointments, visit reports, documentation and notes.
Travel time is not billed.
Are your services covered by insurance?
Loreto services are private pay. We charge a fee for the comprehensive assessment and associated report. All Loreto services are billed by time spent at an hourly rate.
What should I expect during our first meeting?
Our first meeting will consist of a comprehensive assessment of the client in their current environment. This initial assessment will provide the client, family, and care team a “snapshot” of the client’s current health status, functional status, daily challenges, safety, and any areas of concern.
A personalized plan of care will be formulated with the information obtained during the initial assessment. The plan of care will serve as our “road map” as we set priorities to meet our client’s unique needs and wishes.
How do you handle sensitive conversations with families about a change in a patient’s conditions, end-of-life care or hospice options?
The conversation about transitioning a client to palliative care or hospice is oftentimes a dreaded one for clients and their families. Loreto’s goal is to lead respectful end-of-life discussions and arrange for care/services that are concordant with our client’s stated goals, values, and preferences. We aim to preserve our client’s dignity while respecting what matters most to each individual at the end of life.
